I did this but I get the following error: Flow save failed with code 'InvalidTemplate' and message 'The template validation failed: 'The repetition action(s) 'For_each' referenced by 'inputs' in action 'Select' are not defined in the template.'.'. Any suggestions?

Thanks for sharing this! I'm having trouble completing this flow. When I try to drag 'Select' outside of the 'apply to each' loop, I get an error message "This action cannot be dragged out of foreach as it depends on an action inside foreach." I've tried removing the value from the Apply to each output and then dragging the Select step outside of the loop, however this is returning an error as well.

what if there is a third option , Approved with Comments where a person attaches commented Document and send it further to a manager for final approval , please i have three document statuses, Approved, Approved with comments and Revise & Resubmit. in each case the reviewer needs to attach a comment sheet( supporting evidence to why they have selected the status) , if you could show how we can attach a document in each step would be appreciated

This was a great feature. It is now behind a corporate paywall you cannot individually unlock. There are a number of free / available to buy alternatives though clearly not integrated. If you still decide to go with team meetings find your own solution.... My android google translate it is for now...
